Our belief as a Hospice is that individuals should have the opportunity to prepare for the end of life in a way that is physically, emotionally, socially, and spiritually supportive. Hospice provides compassionate care for the person and family so that they may live as fully and comfortably as possible. Continuum Hospice Care/Jacob Perlow Hospice is a caring community that provides interdisciplinary services and skillful symptom management. We are committed to provide quality care to persons living with advanced illness and to enhance community awareness and knowledge of end-of-life issues. Our intention is that with increased awareness of the needs of all individuals facing the end of life, health care systems will respond utilizing the principles of Hospice and Palliative Care, and ultimately promote a more humane society. The goals of the hospice program are to keep the patient as comfortable as possible by relieving pain and other discomforting symptoms and to reassure the patient and family by helping them to understand and to manage what is happening. The focus of care is in the home, because most patients prefer to be in a familiar setting. Short-term inpatient care is provided in the hospice units at Long Island College Hospital, North General Hospice and Beth Israel Medical Center. At home intermittent care is provided through a specially trained team that provides medical and nursing care, emotional support, information, instruction or practical assistance as needed. The team includes physicians, nurses, social workers, counselors, clergy and volunteers. A member of the team is always available by telephone 24 hours a day, to answer questions, discuss concerns or handle problems.

Continuum Hospice Care believes that individuals should have the opportunity to prepare for the end of life in a way that is physically, emotionally, socially and spiritually supportive. Hospice provides compassionate care for the patient, and support for their family and caregivers, so that they may live as fully and comfortably as possible. Interdisciplinary services and symptom management are provided by a team that includes doctors, nurses, social workers, chaplains, volunteers and bereavement counselors. The team works together to help patients and their loved ones with their physical, emotional, social and spiritual needs. Hospice care is delivered to the patient at home, at a hospice inpatient unit or residence, at a long-term nursing facility or a participating hospital. The majority of patients are cared for in their own homes.
History:
Ninety percent of our patients receive care in their own homes, but if that is not possible and the patient does not require the acute care available in the Jacob Perlow Hospice inpatient unit, the Mollie and Jack Zicklin Hospice Residence is available.
