The South Bronx Health Center for Children and Families (SBHCCF), a program of the Children’s Hospital at Montefiore, is a unique, family-centered community health center serving low-income families in one of the nation’s most underserved, at-risk communities. SBHCCF is committed to providing top-quality health care and expanded services to New York City’s neediest families, including mental health services, health education and promotion, nutrition counseling, and developmental screening.
Description:
The SBHCCF provides comprehensive primary care services to children and adults; specialty referral management through scheduling, transportation, and outreach assistance; women’s health and prenatal care; comprehensive mental health counseling; case management and entitlements assistance; complete nutritional assessment and counseling with a nutritionist; group and one-on-one health education; services for people with HIV/AIDS; specialty and inpatient care at Montefiore Medical Center and the Children’s Hospital at Montefiore; and extended hours, including early morning, evening and Saturday appointments, as well as 24 hour, 7 day a week access to medical providers. Through collaboration with other non-profit organizations, the SBHCCF is also able to offer a variety of free services to its patients, including legal assistance through the LegalHealth division of the New York Legal Assistance Group.
